Stories for Care Patient Experience Toolkit
Developed in partnership with the Patient & Family Care Center at Michigan Medicine, alongside colleagues at the University of Michigan, the Stories for Care Toolkit was created to address a core challenge: moving beyond identifying problems to actively designing and testing solutions alongside the stakeholders who own them.
Grounded in real stories from patients, caregivers, and staff, the toolkit provided team members from the Patient and Family Care Center with a structured, repeatable process for surfacing unmet needs, co-creating interventions, and scaling what works. It includes hands-on methods, templates, and guidance to support discovery, design, and reflection—making human-centered innovation part of everyday clinical practice.
